I asked my neurologist PhD/NP re palliative care and he said ongoing medical treatments may be discontinued if I’m in palliative care. In particular, he was concerned about Botox for migraine being discontinued.

I think you should get advice from a MD/DO trained neurologist. PhD/NP is not an appropriate professional to go to for neurological disease treatment.
