The TARDIS has the ability to change where the entrance doors take you, in this case the doors led to the swimming pool allowing her to dive right in.

Clearly the Doctor doesn't give a toss about Grace, Graham or Ryan O'Brien. River falls off a building, the Doctor catches her in the swimming pool. Grace does and ... splat.

Fixed point. The Doctor saw Grace hit the ground and die, going back and changing it risked creating a paradox. The Doctor saw River dive off and got on with his business, knowing he could go back and catch her later.

@@Althalus2010 Tesselecta. Ganger. Zygon. Auton. Solitract. The Doctor could have enlisted any of them to avoid a fixed point. Or she could have used an Extraction Chamber to animate dead Grace, same as he did with Clara. Or she could have taken her Grahame back to spend his final years with Grace in the period prior to Grahame's original meeting with Grace. Or ... there are literally dozens more ways the Doctor could sort this out if she cared to. She simply doesn't care to.

The Doctor always has a way out of any trap. And we delight in finding out how ...
