In order for parathyroid tissue to be preserved during thyroid surgery, its anatomical integrity must be ensured. For this reason the parathyroid glands should always be identified at the time of thyroidectomy. In order to maintain their blood supply, it is important to ligate thyroid arteries as close to the thyroid gland as possible, distal to the origin of parathyroid vessels.

The inferior thyroid arteries supply the parathyroid glands via its branches (supplying both the inferior and superior parathyroids in most cases).
