@@DrAndreaFurlanjust found your channel. For 7 years I suffer from MPS. I have almost constant muscle tenderness and once a month I have tender knots like a bean that is painful and it radiates. I learned to live with them but when it's really unsupportable I take 1 pill of piroxicam 20mg. I will watch every video and do the exercises.

Dear Dr. Furlan, As a doctor myself living with myofascial pain syndrome for 5 years now, initially due to poor posture, I must say you really amazingly summarised all that I have slowly learned about my condition over the years. Through many trials and errors I have come up with a good plethora of exercises and stretching routines i do every second day that have helped me go from 5-8 VAS pain to now 2-5. So definitely my life quality have improved tremendously but still there are some days that are harder than others, and I must say that I would not wish chronic pain on my worst enemy, even though I dont have any :).. Pre my situation I was a really energetic and outgoing personality type, but now I am more of a leaned back, normal energy level, type - and not actively seeking social engagements because of being unsure about what my pain level will be that day, and it can be a hard time being out for a day if it is one of the worse days. I still do social activities, so i dont hold back, but it sometimes just hurt and i live with it. I am by no means depressed, and I love my life and family, and I always focus on the bright side and am so happy that my life is not as bad as it used to be earlier in the pain level, and I am otherwise than my chronic pain healthy. I generally just wanted to thank you for spreading awareness about this terrible underdiagnosed condition, and you really hit the nail on the head in describing everything about it, superfiscially due to time constraints (e.g. who wants to watch a 1 hour youtube video). Your patients are lucky to have you and I wish I had seen a doctor as knowledge full as you about my condition 5 years ago to get me on the right track instead of me having to find out myself throught trial and error which exercises and triggerpoint therapy modalities worked/made it worse for me over 5 years. I have one question I hope you can help me with: You mention short term treatments with muscle relaxants is a possibility. Which specifics ones do you have good experience with, how long do you treat and in which dose, and are there any monitoring requirements (e.g. blood samples). The specialists I have been to have only had inflammatory rhemautology diseases in mind and just checked that i didnt have that, and that is nice to know, but that didnt help, that was physical therapy mainly through youtube since the therapists i have seen just had some general exercixe suggestions where some worked and some didn't, but with youtube physical therapy that is like getting suggestions from hundreds of physical therapies all at once and then keeping what is working and discarding what doesnt :) - and it can be hard because sometimes what is working initially hurts but then slowly makes things better. But stretching, range of movement exercises and full body body weight exercises is what is working know and then a general 100% focus on good ergonomics, since a bad posture hurts imideately. This was quite a long ramble but I just wanted to tell my story to a colleague who really gets it, express how impressed I am, and ask you that one clarifying question. Best regards from your fellow colleague who pretty much have the same understanding of the condition as you do by living with it, and improving it by doing many of the things you talk about. Keep doing what you are doing!

I am glad I found your videos on myofascial issues. I am a rather fit, 60 yr female, love to be active, whether building stone walls, hauling yards of dirt, running or kayaking. For me, I tore the rectus abdominus in a kayaking incident in the summer of 2020, by trying to throw myself into reverse to pick up some plastic in the water and now nearly two years later, been working on it off and on. I have a weak link in my body as just on the "other side" of this area I have a herniated disc and trigger points in that back area. So, trying to figure out how to strengthen front and back to help both. Now in the worst pain of my life, I am watching everything I can and making a document for myself, re: exercise, diet, modality treatments etc. So, a big thank you for such an organized way of describing all that pertains to this issue. I have now watched a few other videos of yours and they all bring more things into clearer focus. Thank you!

Stretching and massage usually make things worse so I really felt hopeless, but knowing that there are experts working on the problem, and that it is possible to treat it makes me feel waaaaay better!

Thank you for your video. It was indeed informative and precisely elaborative. Doctors diagnosed me with muscle spasticity and ulnar neuropathy initially and prescribed me Baclofen. After ingestion of Baclofen for a year, I have been suffering from back and neck pain to the extend I was not able to sit or stand for more than 15mins and my hands lost their functionality. I pretty much became bedridden for a year now (while doing my Master thesis). Finally, after consulting N number of doctors, I was finally diagnosed with Myofascial Pain Syndrome and I have been taking Amitriptyline along with manual therapies like ultrasound, and dry needling techniques. It's been almost 3months into dry needling and I am doing well for the first time in 2.5 years. Now I have increased my workout routine from yoga to yoga+cardio+weight-training. I hope to get well soon enough to continue my research career.

Thank you for your comment. My hope is that many people with myofascial pain will watch this video and show to their clinicians. We need many more healthcare professionals (doctors, physiotherapists, chiropractors, massage therapists) who understand what is myofascial pain and have the skills to examine and to treat people with this type of pain. It is one of the most common types of pain, and so easy to eliminate if treatment is started early in the course of the disease.

It is never too late to start treating #myofascial pain. I had patients who had pain for many years and they still got benefit. It may take longer and more effort, but there is always hope.

I understand your question now. Yes, pain (such as myofascial pain) can change the pain system (neuroplasticity) causing the condition of central sensitization (that can in turn become fibromyalgia). Never been officially diagnosed with MPS but I’m pretty sure I have had some version of this all my life. When I was a kid I would get bad shoulder and neck pain and big muscle knots constantly. In college it got worse and during grad school I had several flare ups that had me totally incapacitated (in a neck brace, lying on the floor, crying). For a long time I thought everyone must be in pain all the time and were just better at dealing with it than me. I am healthy and active, I studied ballet from childhood through college and have practiced yoga for nearly 20 years, but I suffer from severe DOMS if I’m not careful. Just having a name for this chronic pain makes me feel validated. For those just starting out on this journey, an anti-inflammatory diet really helps. So do supplements like magnesium and potassium, as well as drinking lots of water. I have also had great results with a combo of trigger point acupuncture, chiropractic, physical therapy, and strength training in the gym (start slow and gentle). Good luck everyone, it’s a marathon not a sprint.

Dear Mother May I, I find that many of my colleague physicians do not even believe that muscles hurt. They don't examine the muscles because they feel embarrassed by not even knowing the anatomy of the muscles. It is a deficiency in our medical schools and residency programs. I'm trying to change that. Physicians who see patients in family medicine, emergency medicine, orthopaedics, they all need to know that Myofascial pain exists and it is real. I'm a physiatrist, and we learn this in residency. Thank you for your comment.

Dr. Andrea Furlan I am so grateful that you made this video and that I found it. As a woman, I tend to avoid going to the doctors for this pain (or any pain ever) because I don’t want them to write me off as exaggerating, tell me to lose some weight, or presume I’m just seeking pain meds. It’s happened before so many times and it makes me feel awful and embarrassed. I’ve never really had good experiences with doctors as a patient. (I work in healthcare and my colleagues are great, though.) I went to a physiatrist once for this pain and he spent less than 3 mins in the room with me, did not examine me at all and told me he couldn’t help me and that I had to see a chiropractor. So I went to one for a year to get adjustments and ultrasound therapy, all of which did nothing for me. Massage seems to be the only thing that helps, though I always feel sore and bruised after them and it takes me a few days to feel what I consider “bearable” muscle pain. It never goes away completely. I’ve never met a doctor who has even taken myofascial pain seriously, never mind thoughtfully articulate what it is and the treatment for it the way you did here. I grabbed my back as I leaned over to put on my shoes. I was taking anti-inflammatory drugs and muscle relaxants. It helped. As soon as I stopped taking muscle relaxants, the pain spread to my leg. And already tormented for more than a month. Massage and stretching slightly exacerbates the pain. The doctor prescribed duloxetine.

I've been suffering with this or fibromyalgia for two years but wasn't sure what it was. With the help of chat gpt ai and this video I am finally narrowing it down to a diagnosis. I described my symptoms to Chat gpt and it gave me the term myofascial pain syndrome. I then searched for the term on youtube and found your video and I knew when I saw not only MD, but also Phd after your name, i knew I would be getting some good information. You did not disappoint. Dear doctor, I cannot tell you how validated I feel from listening to you say these things. I have been working to get the kind of help I need for many, many years. I have done so much reading and listening, and also, sad to say, begging doctors to give me the kind of tests and treatments you are talking about. I cannot find one physician, no exaggeration, who will touch me with their hands. They touch a few places, and they just wash their hands of me and say the word, fibromyalgia. But you have confirmed, at least to me, my condition. I don't understand why it has been such a tremendous challenge for me to find a physician of any kind to really talk to me about my symptoms. I even present them with a list of my symptoms, categorized by system or region. I'm helping them help me. But they ignore it. They are hell bent on their own protocol and so far have been completely resistant to some basic tests that I am nearly certain that I need. Now to find out where you are located. Because maybe I need to come see your office.

@@DrAndreaFurlan All of this started with my neck Pre-COVID (Nov 2019), then started affecting my tharasic area, tightness burning in my traps and mid shoulder blade area. Lots of knots. I felt like I needed my spine adjusted everyday. Then what started happening is I started to get nerve pain moving around my body. Like pin pricks and sometimes but less common nerve shocks as well as twitching, usually at night. These all happen in random places around my body but don't happen all the time. However it usually only happens when my neck or upper back are bothering me. Could this random nerve pain be caused by MPS? My chiropractor believes so but my neurologist isn't sure. It causes some anxiety, the not knowing. Whenever I do relaxation techniques whether it be: deep breathing, stretching, massage therapy, acupuncture or chiropractor treatment the nerve pain and all my pain goes away. It usually comes back a day or two later. If I take a muscle relaxer or Tylenol or Ibuprofen it also goes away. After ten months I'm now starting to go to acupuncture and chiropractor twice a week. I went to PT early on but my appointments where so far apart I felt like I was starting over every time I went back. Anyways you video makes me pretty convinced this is what I'm dealing with outside of the random nerve pain. Thoughts? 🙏

hmm.....i started with myofacial....yet i was never told that is what it was. Is this also something that most doctors know little to nothing about? I now have full blown fibro. All the triggerpointing and TENS in the world no longer work. I do not have a good response to medication. I used to live an active life, but now i can barely move. And yes i understand the difference between muscle fatigue and pain. I used to love the feeling of fatigue occasionally after a particularly tough training session. It made me feel good. Now i am mentally exhausted from pushing my body to do things it doesn't want to, or hurts too much to do. I used to be mentally strong, now i cry at even the littlest things. I'm sick of it.

See if you can get pharmocogenetic testing to see which antidepressant or pain meds that you may have the least problem with. As a patient , this has helped . Suggesting that you research this and discuss w your pain specialist or psychiatrist, if in the US

@@DrAndreaFurlan Ja i have seen that i had burning processe on my left side my nevrolog Dockter gave me 3times 300 milli gram a day and I took Mri Brain and spinal it does'nt show anything my My Brain was fine even my spinal too Gabapentin took away the burning straff insted i got kronisk back pain she told me to take 600 mg gab in the morning 300 att lunch and 300 in the evenning and almost a month since i started and I informed i have no Nevro problem.

Been dealing with MPS for years. I use SSAR exercises consistently, heat pad, tens machine, dry needling, Botox trigger point injections, meditation, yoga, everything you motioned. I’ve been in a pain clinic and been in multiple complex rehab programs. Temporarily relief happens but nothing more than that. Makes me wonder if this is just the way I’ll feel for the rest of my life. 😪 I have stomach problems (gerd) from all the NSAIDs. Every anti depressant you name didn’t work it gave me horrific side effects. What a nightmare! I have quite the tool box myself

Hi Ronnie-Lynn, I am sorry to hear about all your attempts to get rid of this pain. I know it can be very frustrating. Many of my patients find ways to adapt to their condition and enjoy their lives, despite still having some pain. Life is a blessing and each day is a gift. We all need to learn how to find meaning and purpose so we can focus on what is important and do not let our symptoms interfere with our goals --> I am basically preaching to myself here.

Found myself back again, watching this video. It seems that I cycle through a year of some improvement and then somehow end up spending a year suffering where of my quality of life takes a hit drastically. I’m in a season of being in constant excruciating pain. I suffered a neck and shoulder injury. More than 10 years ago. So much muscle atrophy and deterioration weakness and shortening of the muscle. I’m gonna go and do some more physical therapy and dry needling and another complex program with some CBT to see if that will help reset the pain cycle again like it did a bit in 2018. 🙏🏻🤞🏻

All this generosity in providing medical scientific information comes only from a great humane heart that a woman like you can have it👍👏😍💖❤️💐💐💐 Actually I’m having right side shoulder pain for one and half years, it gets worse during sleep and every night my sleep is kinda torture. I got many Physio therapy sessions and my physiotherapist worked on my shoulder with some movement and also gave me some exercises but it just made it worse and worse and ended up sever limited range of motion so that my hand does not go back higher than my hip😔😖 I went to chiropractor as well but didn’t get result just increased pain and motion limitation. At the height of my frustration, I saw your video (exercises for shoulder) and felt better after doing only two or three exercises and my pain was slightly reduced😀🙏❤️🌹🌹🌹 I’m going to do these exercises and I hope one day I will be free from this tribble shoulder pain☺️

@@DrAndreaFurlan Actually my family doctor who is an angel in my life, knows from the beginning and I have had two ultrasounds so far., the second one showed supraspinatus tendinosis. Recently she referred me to sport medicine. He suggested me one exercise that if my pain does not go away he will inject steroid and that exercise with strip doesn’t work.
