Palmoplantar hyperhidrosis may be either continuous or phasic. When continuous, it is worse in the summer, and not so clearly precipitated by mental factors. When phasic, it is usually precipitated by minor emo tional or mental activity, and is not markedly different in summer and winter. The hands may be cold and show a tendency to acrocyanosis. Sometimes the sodden skin is also cold and cyanotic. Palmoplantar hyperhidrosis is one component of various syndromes in which palmoplantar keratoderma occurs.

Investigations(Tests) 1.Thyroid function and 2.gravimetric determination of sweat rate estimation may done. Treatment:- 1.Topical 0.5-2.0% glycopyrronium bromide cream For Feet:- 2.Formalin 1% soaks 3. Glutaraldehyde 10% in a buffered solution swabbed onto the feet three times week For hand and feet:- 4. iontophoresis, using either tapwater or 0.05% glycopyrronium bromide solution.Direct current is usually used, with each palm or sole being treated for 30 min with 20 mA, initially three times a week. Once euhidrosis is established, once a month maintenance done. 5. 0.1 mL of diluted botulinum toxin by intradermal inj. can be given to 1 cm² areas of skin appropriately regional nerve blockade. Each hands usually requires 20 and each foot 24-36 injection. A reduction in sweating is apparent within 48 h and the benefit will normally last for up to 8 months in axillary and 6 months in palmar
