Specific Lab Tests - Thyroid Function Tests
Thyroid-Stimulating Hormone (TSH): Typically elevated in Hashimoto’s thyroiditis, especially in hypothyroid stages, as the pituitary gland compensates for low thyroid hormone levels.
Free Thyroxine (Free T4): May be normal in early stages or subclinical hypothyroidism, but often low when hypothyroidism develops.
Triiodothyronine (Free T3): Less commonly affected but may be low in overt hypothyroidism.
Specific Lab Tests - Autoimmune Markers
Anti-Thyroid Peroxidase Antibodies (Anti-TPO): Elevated in over 90% of patients.
Anti-Thyroglobulin Antibodies (Anti-Tg): These are also commonly elevated in Hashimoto’s but are less specific and sensitive than Anti-TPO.
Thyrotropin Receptor Antibodies (TRAb): Usually negative in Hashimoto's but are tested if there’s suspicion of overlapping Graves’ disease.
Additional Lab Tests:
Erythrocyte Sedimentation Rate (ESR): May be elevated due to inflammation.
C-Reactive Protein (CRP): Typically normal but may rise with active inflammation.
Normal Thyroid Measurements:
Isthmus Depth: Less than 0.5 cm (approximately)
AP Length (Lobe): Less than 2 cm (approximately)
Length (Lobe): 4-7 cm (approximately)
Width: 2.5 cm (approximately)
Acute Diffuse Hashimoto Thyroiditis:
Diffuse enlargement of thyroid gland
Heterogeneous appearance of entire gland
Numerous hypoechoic nodules (Giraffe Pattern)
Diffuse hypervascularity on color doppler (Thyroid Inferno)
Hyperechoic linear strands
Chronic Diffuse Hashimoto Thyroiditis
Small atrophic thyroid gland
Heterogeneous appearance of entire gland
