This is the Clarinet Polka - Dziadunio or Polka Dziadek (Grandpa-Polka) are other titles of this polish melody, as I read - folk composition from the end of XIX century.

I have this on a 45 rpm record by Kenny Ball and his Jazz band and it is called Tyroleon stomp but is this exact tune.
