A scoop is used for wax. These are ribbons of dead skin, a condition called keratosis obturans. You can tell by how it appears to be "encased" in a white, puffy layer which is dead skin that has not oxidized yet. The darker center is full of very tightly packed ribbons of dead keratin. He's slicing thru the ribbons with the hook tool so he can suction them out.

I hope this person comes in every three months or so till the ear can clear itself naturally or it's a quarterly cleaning. I am sure their hearing return was great after the pain and itching subsided. Great job on a tough clean out. I think you speeded up the video at a few points, and it took longer than 15 minutes to clean all that keratin, then the surrounding skin in there extracted. Very good work.
