I have a similar dress and yes you do, because there are built in shorts. It's basically a bodysuit (without snaps) underneath.

The athletic dress is all fun and games until you have to get naked to pee and your toddler knows how to open the stall door 😳🥴 I did just see one on Amazon that has shorts but they are separate which is ideal for me having lived through these situations 😅
