Thank you for simplifying this very important but confusing topic. I have a question regarding the ‘Posterior MI’ section: Why don’t we call it NSTEMI (ST depression in anterior leads). When do we suspect those are reciprocal changes and not true ST depression (subendocardial ischemia)?

I don’t know what Dr Alrahamneh thinks here 😅 But I will venture to say that in the right clinical context (active chest pain/concerning history) you should definitely worry about posterior wall STEMI and to that 15-lead! Such a simple thing, not expensive at all, takes 5 min, may save a life.
