I had right hip arthroscopic surgery FEB2021 - I experience spasms and various pains in my anterior thigh (sharp, burning, allusive itching, cold wash, tingling), with other groin, knee, inner thigh, outer-hip and buttock pains. I have finally gotten my pain management team to schedule an EMG/NCS. PM diagnosed me with meralgia parasthetica- however I have been pushing for the EMG for almost a year, now, due to muscle loss and functional loss. (I cannot fully extend through a step while walking without my leg buckling and so I walk with a cane). This information gives me some comfort knowing what I am going into and the science behind the method. I have had spinal nerve ablation, LFCN ablation, corticosteroid injections and I am scheduled for a steroid injection for my sacroiliac joint. They’re trying to find the pain source - but I truly feel it’s femoral nerve damage from the surgery. Thank you for this video - It’s been a hard road and I’m hoping for some answers with this study.

I did some research and in turns out fibrillations are due to muscular denervation, not necessarily myogenic diseases. The reason you get the fibrillation is because the denervated muscles are more sensitive than normal muscle. I guess you can make the argument that in certain inflammatory myopathies there are muscle fiber break down and mild denervation causing occasional fibrillations.

I am getting an EMG in a month due to my muscle spasms/ twitches. I have both fasciculations ans I think fibrillation. I can feel a buzzing/vibrating feeling in my muscles sometimes.

Hi, Let’s say a patient has BFS. A neurologist puts them under the EGM test. And connects the electrodes in the areas that the patient is experiencing twitches. During the EGM process, twitches happen obviously, at the patient is anxious and unlucky. How would the EGM result show the graphs during all three categories? Especially, in the first scenario where there is no contraction. Does EMG show some abnormal activity since the muscle was fasciculating during the test?

acetylcholinesterase is not attached to the nerve side, but muscle side. Muscle side also produces it. Therefore, destruction of the nerve will not results in a decrease of acetylcholinesterase

Fasciculations can occur outside of neurogenic disease . During autonomic nervous system dysfunction mitochondrial biogenesis is impaired, acetylcholine leaks from the neuromuscular junction and sarcomere contractions occur.

In denervation there's increase in amplitude on EMG due to compensatory hypertrophy. But on NCS, CMAP is reduced. So how do both the results co-relate? Also, on EMG can we differentiate axonal vs myelin injury and if so how to do it? For example in SMA

I had a crush injury to my hand 6 months ago, no broken bones, had tenosynovitis on an ultrasound, had an mri with structural problems. I’ve had pain in my hand the whole time early crps was a diagnosis until I had an independent medical assessment and was told I had guyon tunnel syndrome, for the past 3 months my arms been slowly getting sore and my shoulder and neck are effected aswell. I’m on anitryptidline but I have a feeling the nerve is just damaged, is that possible?

My EMG results in abnormal high amplitude in motor unit potentials (more than 2.5 times). Does it mean I have neurogenic disorder? Some sites mention that high amplitude could happen when there is re-innervation at present. You also mentioned in the video when one nerve lost its habitat, the surrounding nerves could take over. Mean there is hope for the muscle to work again. But then some sites say neurogenic atrophy couldn’t be reversed. I am stressed because I don’t think my PCP approach my matter with the right angle. And it takes months now and I’m in the middle of nowhere. The window frame is narrowing. Could you help me please?
