1. b is correct. If you said "a", No - this is a way of adding canvas apps into Microsoft Teams, but not model-driven apps.
2. a is correct.
3. d is correct. Tab apps are inserted in Microsoft Teams. Personal apps are Added to Teams from the Power Apps portal.
If you said "b", No - that would be the case if I started in Microsoft Teams, when to a channel, click on +, select Power Apps, and then an existing app.
