Thank you for this case presentation. The angiographic results are superb! I did not get the use of impella in this case. From the echo imaging it seems to me that the inferior wall was either hypernating or scar..so I was wondering if there woud be any hemodynamic collapse in case of flow impairment in rca..

Distal embolization is always a possibility as you mention - the operator has to always determine whether the value of the information obtained from IVUS exceeds the potential risks. Thank you for your comment!
