While it is technically possible to also do it with blush and similar things, it is very unlikely due to several technical and UX issues and circumstantial evidence (like Matsuri's model presentation) hint against that. If you have the impression that her blush is always perfectly timed, then either she is skilled in handling her live2d (more likely) or it's indeed automatic (less likely). In the end, the goal is usually to convince the listener that it's automatic anyway, so goal achieved, I guess. :)
