hello doctor m in 8 months after treatment for epididymitis orchitis that was very swollen and inflamed i think i had some pus in thunica vaginalis from what i remember ... Now pain goes up to spermatic cord and to the prostate and i have problems urinateing...now my question is ,from your experience what happens whit a severe case like mine ?ultrasound says my right testicule is fine but i feel pain and epididymitis is tickened after inflammation/infection and i can feel thunica vaginalis being larger around my testicle ..can there be recovery ? thank you

Which country are you living in? Have you seen a urologist? Have you ever had a sexual health screen? If also getting penile tip pain and increased frequency and all tests are negative and ultra sound scan is normal then need to consider kegal exercises for chronic pelvic pain syndrome. I did a video in this a long while ago. Give it a go if everything else is negative.

Hi, Sir. I have been suffering from heavy pain and swelling in both testicles. I consulted a specialist. In USG Scrotum test nothing found . Doctor told me that there is some bacterial infection. He suggested me to take Chymoral Forte, Acemiz-S, Oflox-OZ, Rantac -15omg. I have been taking these medicines from last 25 days. But the problem remains the same . I was consulted urologist 2 days back and he suggested me to take only Chymoral Forte once a day and Doxyxycline 100mg twice a day after the meal. I have been taking doxycyline for 10 days now and symptoms such as tenderness, redness, scrotal sac size, lower abdomen tightness, frequent urination has gone away, however the size of my affected testicle is still almost the same (has maybe decreased slightly). Do most testicles go back to normal size within a couple weeks? Mine has been 2x the normal size for about a week. My tests confirmed I had a UTI and it has given me this epididymo-orchitis. Doctor is sending me for an ultrasound just to rule out testicle cancer but im guessing its reasonable to assume it isnt cancer because it all coincidentally happened when I got a UTI? He just seemed a bit concerned it was this size and kind of hard. is it ok if I take my open my doxycycline capsule and eat in with a tiny bit of apple sauce and water in 1 quick gulp? I have a problem swalloing pills and was told this was ok by a couple pharmacists. Is it ok to have sex once its confirmed the infection is gone? Obviously not too rough.

Hi there, sorry for the late reply. Once you are better and feel up to it you can have sex but if you was diagnosed with EO then your partner should have had a 1 weeks course of doxycycline as well. If it was a non-sexual bacteria causing this problem then you may get that bacteria back again. Depends on the make of doxycycline, some can be broken some can be dissolved, I'm happy to go with the pharmacist on this one. Testicle size should return to normal in a few weeks, doctor did the right thing and sent you for an USS. How did the USS go? Kind regards Dr Bret Palmer

@@DrBretPalmer Long story short, uktrasound showed it was a hydrocele I have as well as swollen epididymis. Perhaps no orchitis (swollen testicle). The 2 weeks of doxycycline helped but didnt get rid of it and it came back worse and i ended up with a fever and had to be admitted to the hospital where they put me on IV of antibiotics (cefixime i think?) and antiinflammatory for 5 days. I am now at home on oral antibiotics of the same kind for 10 days. size, swelling has inproved now after the hospital stay. Am I understanding you correctly that i should avoid any sex with my wife even if my infection seems to be going away, because she may have been carrying this bacteria in the first place that gave me this whole problem, until she has taken a week of doxycycline? Is it possible she has been giving this to me once, or repeatedly, yet not be affected by it herself?

Hi doc! I'm being suffering from bilateral epididymitis for last 4 months. All the urine test and STI tests were clear. But the swelling is keep on developing. My gp prescribed amitryptiline to manage pain. He is telling me not to worry as ultrasound shows no issues. But I'm greatly worried due to recurrences and the swelling not arrested yet. I'm not yet married and it greatly affecting my day to day life. Not sure how worse it will go. Why am I keep on get swelling even though no infection? Still I don't have any urinary complaints apart from this swelling and pain. Help me to get a way out

@@DrBretPalmer Only epididymis keep on swelling. No colour change or irritation in skin. all the veins rear to testicle thickened. but in right side not that much as in left side. i have checked with gum clinic and they said i don't have any STI so i need to follow my GP.

No pain when passing urine. No pain on ejaculation. And testicles non tender. You’ve had a SHS and all clear. USS was also clear. The lesion that is swelling, is it all over the back of the testicle or a small smooth lump? It could be a cyst? May need a repeat USS and referral to a urologist. Also where do you live? If you live in an inner city area or have come from an area which is high in TB? I'm not saying its TB but there is a wide list of possible causes. If you are fit and healthy with no problems then may need reinvestigation. If it is a smooth hard lump and joined to the testicle maybe worth doing a pregnancy test... yes I know you are a guy, but helps to rule out a seminoma (rare but these lesions produce the pregnancy hormone and if big enough can turn a pregnancy test positive). If lesion very soft then not likely. In general, try to; Lie down so that your scrotum is elevated Wear an athletic supporter (briefs will do) Avoid lifting heavy objects Avoid sexual intercourse. Let me know how you get on. Bret

@@DrBretPalmer TB test already in progress. next week expecting to get result for 3rd sample. already 2 samples came negative(Totally 3 collected). it becomes tender whenever recurrence happen. No cyst observed in palpation and from previous USS test. USS test 2nd time referred and awaiting for appointment. My GP told that we cannot refer urologist with this data as all came clear. so told me to wait untill new USS.

Hello Dr really appreciate your help with this subject. I am 4 days into a 2 week course of Doxycycline but I am getting severe psychological side effects (overwhelming depression/anxiety/suicidal urges). I don’t think I can continue. Is there no way to recover from this without antibiotics? Can I try strengthening the immune system another way? Thank you.

Antibiotics are a good way of resolving bacterial infections. However if you are getting suicidal thoughts and you want to act on them please go to your emergency department now and tell them. If they are only thoughts and you are not going to act on them then you must contact the clinic who gave you the Antibiotics. If you think its the Antibiotics giving you suicidal thoughts then stop taking them and speak to the clinic who gave you them and get alternative Antibiotic treatment.

Hi Doc, great video very informative! I have had epididymitis at least three times in the past, all treated with antibiotics and it’s gone and then a few months later it’s back again .. this time the pain is excruciating (lower left side of abdomen radiating down into the left testicle, the testicle is fairly painful and tender to touch, not just the epididymis, the whole thing. This time the nurse says it’s epididymitis orchitis, which may explain why the pain is far worse than I’ve had previous. I was put on antibiotics (doxycycline) for 10 days, One tab twice a day. The pain eventually subsided after the 7th day into the antibiotics. 4 days after finishing the antibiotics I am experiencing some discomfort, a week after finishing them I am in severe pain again. The doctors have booked me in for a CT scan. I had a blood test a day or two after the infection started as the doctor said he wanted to check my blood as I keep getting reoccurring infections ... the blood test came back as a low blood count, he said this could be due to the infection so he has booked me in for another one. That would be the first time I’ve ever heard of a low blood count, I am a regular blood donor (before COVID). I do wee a lot but at the same time I also drink a hell of a lot of water during the course of a day, I workout regularly but I can’t at the moment due to the severe pain I am in. I have no history of any STI’s I visited my local icash clinic last year confirming this. I have the CT scan coming up but any professional advice you could possibly give me would be fantastic, while I trust in the health service I am worried that there could some underlying condition. Thank you for you comment. Usually the standard course of doxycycline treatment in the UK is 14 days of doxycycline and if there is enlargement of the testicle an ultrasound scan is then performed within two weeks if all STI tests, urine cultures and pregnancy tests (some testicular cancers give positive pregnancy tests) are all negative. If your testicles are their normal size then a CT scan would appear appropriate. If you have a sexual health clinic near you then I would advise you visit them as they can run tests others may not think of, e.g. mycoplasma genitalium. Please also remember dont wear boxers but wear briefs to support and rest the testicles. Let me know how you get on. Good luck

As long as the testicles are supported. Stick with swimming as the water supports the weight of the testicles. I would leave off of cycling, jogging and weights until you are better.

I had my first attack of this at age 17 from a prostatic infection. Now 83 and it’s recurred every so often. It’s impossible to get rid of although occasionally treated on bad occasional flare ups with antibiotics. . Exacerbated by having a vasectomy SO DONT DO THAT ……intermittent infection of prostate ( remember antibiotics cannot get into this gland ) and back and forth ping pong between there and epididymis for decades. Epididymal cysts shouldn't be painful but if it's growing then get a second opinion and consider getting it removed/investigated with an ultra sound. You may need to see a urologist. Good luck hope you find a resolution.

Hi Amiodarone, Sorry to hear its been going on for so long. Not sure if you have had a full sexual health screen but I would recommend that and a review by a sexual health doctor and/or a urologist. Remember to also support the testicles by using briefs not boxers and don't wear too tight trousers (no skin tight clothes). Your urologist may want to try 2 week course of doxycycline 100mg twice a day for 14 days (stay out of direct sunlight and don't take zinc or iron supplements with this medication). Good luck Bret

Doc , just an update ... I have no testicle on my right side after this bacterial infection and abcess. The lab here said they had never encountered a bacteria of this type. However i have recovered . Thanks for your help! By the way Tramidol is the only pain medication that can get you through this .

I’ve had this for about 4 years now. It’s in both of them and doesn’t bother me during the day, but swell up during maturation or sex. My prostate feel like it swells up aswell. If you have never had a sexual health screen then contact your local sexual health clinic and ask to speak to a doctor. They should then book you in and test you for an STI. If all clear then they will suggest going to a urologist or your family doctor. The process is different in different countries but many countries do have a free sexual health clinic you can contact. If you are not sure what to say then read out what you have just sent me. It states your concerns very well but I would advise a sexual health screen and an examination to make sure everything is okay. Hi Riswan. If you have had a negative sexual health screen (with negative mycoplasma genitalium) and urine cultures are also neg and you have had a 2 week course of doxycycline and nothing has changed then this could be chronic non bacterial prostatitis, also called Chronic Pelvic Pain Syndrome (CPPS). You can get more information from here: prostatecanceruk.org/prostate-information/further-help/prostatitis This website gives a good explanation. You will need a few tests and an Ultra Sound Scan and be under a urologist. Please ensure that the sexual health screen as stated above has been done and is negative. Hope this helps. Dear Dr Palmer, I got diagnosed with prostitus and give antibiotics for 2 weeks Ciprofloxacin and I completed my course and 3 days later I feel pain in my left testical and was swollen and i got told I had EO and i have been give Doxycycline for 2 weeks but I have been 9 days and the pain is there and it still comes and goes. I have about 5 days left on this, will it go away? i am 32 male, i have also got a weak urine stop and start.. i have also been screened for clap, gona, hiv , sif. Hi Malik, You should be screened for mycoplasma genitalium as well as chlamydia, gonorrhoea, syphilis and HIV. Have you been to a sexual health clinic/GUM doctor? Very important to see a specialist in this area. If you live in a country where there are no sexual health services then seek out a urologist with an interest in sexual health. You will also need a prostate check as well. Depends on you medical history but you may need a rectal exam and other blood tests (via sexual health or ideally via urology). As with other posts please use supportive briefs and allow the testicles to rest were possible. That means no jogging or impact sports for a month, go swimming instead, supports the weight of the testicles..... only if you can. Hope this is helpful but you need to see a sexual health doctor and a urologist. Let me know how it goes. Good luck Bret

They did a rectal exam and they said I have prostitus and gave me a 2 week dose of antibiotics, but the I got EO 3 days after I finished course. Now i am 10 days into that course, swelling has gone down in the testical but it still sore, i also feel pain in my groin and thigh. Is this normal with this?

If you got these meds from a sexual health clinic then you can go back to the sexual clinic to discuss the dide effects. If the swelling is reducing then that's very good and you need to complete the full course. However if the soreness is concerning you then you need to discuss these side effects with your clinic. Is the soreness due to your infection as opposed to the medications. Doxycycline shouldn't really make you sore. Call them up and have a chat.

Yes you can get referred pain and also depends how swollen the testicle is as well. It could be due to the EO. Complete the full course of doxycycline and rest the testicles by supporting them with briefs.

Oh, I took your advice, i went to hospital and they told me I had a torsion of left testicle appendage. On my right testicle they said it was perfectly fine although I’m still feeling sharp burning pain which lasts like 2 seconds

You need to be reviewed by a sexual health doctor or a urologist. You still might need a course of antibiotics because it could be a bacterial infection (not all infections are sexual). Also wear briefs to support the testicles.

If you live in Europe go and see a sexual health service, dont try a system you've no idea how it works. If you want to try something from home for BV then you can try natural live unflavoured yogurt, place a small amount in the vagina or alternatively take a strong dose of Probiotics, BioGlan for women is good. Also dont use soap to wash the vagina only water or dermol 200 or E45 shower. Lastly don't wear tight clothing. But if still unsure then go to a sexual health service and ask them directly. It is more than possible for a man to have children with one working testicle. It may take a bit longer but still not a problem. If you find a partner and do not conceive a baby after a year of trying then you may want to get your fertility checked. But this all depends on your age, if you smoke (stop smoking), if you are over weight, how often you try and this also assumes you are fertile in the first place. Find a partner and first see if there is a problem, dont waste money now seeing if you're fertile today if you're single; as you dont know if you will become infertile in say the following year. In my case it recurs from time to time. And I have to take antibiotics, I have been taking antibiotics from 3 to 4 months that is on and off like one to two weeks a month . And all the doctors I go to are so chill and relaxed and here I am with pain . I can't even lift weights now!!!

You may need to see a urologist and consider chronic prostatitis. You should have been checked for all the main STI's including mycoplasma genitalium. If all negative and it's considered by your urologist to be chronic prostatitis then you will also need to do some breathing exercises. Which are: Clenching the pelvic floor muscles for 6 seconds and slowly release as you breath in ( yes must be breathing in) over 4 seconds. I hope it starts to ease for you. Good luck.

ive had this and trust me you dont want this i had a ecoli infection which was caught in a meatworks i was working on a table where i was getting constantly wet jocks due to run off from the table which was mixed with puss and animal shit i first noticed burning when urinating right on the last squeeze to clear urine then it went from there i went to the doc he did a urine test after i explained the symptoms of pain and fever he sent me home with antibiotics that night my god the pain and fever and vomiting got real bad i ended up in hospital for 3 days on a iv drip testicles the size of softballs and 4 months in bed and the doctor getting his ass ripped by the doctor in the hospital over the phone for doing the wrong thing. i really should have sued the doc but it took 6 months to come good there where after effects like trouble getting a erection and a loss of penis size which i have only just fixed and its been 10 years

Hello I am a boy and I got diagnosed with acute Epididymitis, and got given 10 day antibiotics. So my symptoms have gotten way better after 2 weeks. But the thing is that the last couple days I have been feeling a sharp pain now and then in my right testicle too, so now I kinda feel pain in both testicles

I would recommend going back to you're doctor for a review. If the sharp pain has increased slowly then infection may not have gone. If the pain occurred very quickly and you are in alot of pain and the testicle is discoloured then seek medical attention very quickly as you would need to rule out torsion (where the testicle is twisted). If however everything is kind of okay just a bit tender then wear some briefs so that the testicles are supported. But you will need to be reviewed by a doctor again if the pain is coming back and also be screened for a range of sexually transmitted infections. Usually doxycycline 100mg twice a day for 14 days is usually given (provided no allergies). Hope the above helps. Kind regards Bret

Not sure how old you are but I would advise if you are a boy and a kid that you speak to your mum and dad and let them know the problem, it's embarrassing but they may help with finding a doctor for you. If your over 18 then seek out a doctor soon for a review.

Okay. No running sports, swimming okay as supports the weight of the testicle (but no diving). Drink fluids so urine is clear. Wear briefs for a few weeks, if doesn't settle and gets worse, you will need to be reviewed by your doctor sooner rather than later as may need a longer course of antibiotics. Speak to your parents let them know what's going on. Hope this is helpful. Good luck.

Ok guys after 3 months there may be light at the end of the tunnel. After 21 days of Levoflaxin not alot of improvement, then I develop a large abscess on the surface of my scrotum. The urologist then cut it and left the wound open to drain for a week now. Put me on Bactrim for 14 more days. Within two days i felt better . Maybe the abcess indicates the end if it . Lets hope because i cant do this again

Doc I ran out of money treating this condition A month later and i have finished my antibods and i have no pain but my right testicle is still swollen Will it go down with time or is it permanent ? Do i have to have it drained keep in mind i am over $2000 in and have run out of cash... Thanks

What country are you in? EO should be at least 2 weeks of doxycycline medication 100mg twice a day for 14 days. If you've had this and still have swollen testicles then you need an ultrasound. If one testicle has been slowly growing and is smooth (not lumpy) and you have weight loss then a urine pregnancy test is sometimes also performed to rule out seminomas (yes rare but also good and easy to rule out). Seminomas produce Beta-hcg which is a pregnancy hormone. All of the above does depend on a very detailed medical history. I would advise to see a urologist who has an interest in sexual health and can ultrasound the testicle. Hope this helps.

Dr Bret Palmer im in Texas and the doc put me on Levoflaxin for 21 days An ultrasound was ordered again my 3rd by the way at $500 each . Over a month now and still swollen. I feel im being led on .

Hi. Sorry to hear it's not going well. Levofloxacin can be use against some infections. You should be retested for the four main infections just to make sure (HIV, syphilis, gonorrhoea and chlamydia ) but also tested for mycoplasma genitalium. Urine test general dipstick test and beta Hcg (again to rule things out any home pregnancy test will do). I'd request an USS and this should include a doppler and if a mass is seen then may need a biopsy. On the Internet there's a sexual health clinic in Dallas county. 2377 N. Stemmons Freeway, Dallas, TX 75207. No idea if this is near you and I don't work in the US system. UK is completely free..... so if you are visiting. Hope this helps.
