Once you make an error it changes you as a nurse. You're humbled. You're humanized. You're hurt. But you learn so much about trusting your gut, double checking yourself, and I believe it makes you a better nurse ❤

Similar case just happened to me. But it was insulin. I was so scared and did not want to tell anyone. Ended up telling the next shift nurse what happened and she was so cool and supportive about it. She kept an eye on my patient and when they checked his blood sugar again it went up even higher, which was the total opposite reaction of what I expected. Afterall my patient was not harmed. But I learned my lesson and will stick with me forever

Made a medication error today as a nursing grad. Didn’t cause pt harm, but started to fear I might lose my job and freaked out like crazy. I did report it. My Mum to reminded me I need to remember I am human. This video helped me so much. Thank you for sharing your story.

I too am a recent nursing grad and made a medication error on the first day of my preceptorship. I gave 100 mg of cortisone IV instead of the prescribed 50 mg. It happened because I did not have access to the EMR yet and my preceptor just told me to "give all the meds on the bedside table". After I gave them, he asked me "Did you give this whole vial of cortisone?" and I said yes. Then he made me look in his EMR and I saw my error. I felt horrible! But at the same time it kind of was his fault for not specifying the dosage, especially when I didn't have access to the EMR.

I just found out that I forgot to report an important neurological change of a patient to the doctor whilst on shift. I kept running around busy saying ill send a page soon, and I forgot. I felt sick to my stomach when an email went out to the whole ward notifying us that this was policy and poor nursing care. This video has really helped me to understand and feel better about what I had done so thank you. I responded to the email despite feeling embarrassed and I will take this as a lesson to do better in future

It seems to me that as I get more experience I make more mistakes / errors. Thankfully nothing has caused patient harm, but I’m so frustrated and upset with myself! It makes me lose trust in myself. A few times our omnicell has flagged me because what I pulled didn’t match the order- that was easily corrected. I’ve had the wrong concentration for a medication in the Iv pump vs what is ordered. I’ve forgotten to cap a central line before. It’s so embarrassing that 2.5 years out I’ve had these errors/mistakes happen in the last year. I’ve debated sitting down with my manager to talk this all out but I’m not sure if that would help or hurt me. Obviously she’s aware of them. There is so much to pay attention to every single shift! Working harder each day! Thank you for this video.❤

It’s so nice to be reminded that the level of perfection we expect of ourselves is unrealistic (although high expectations of our responsibilities and role as a care provider is good to strive for). I had been in nursing for a couple of years when I was going to give an unusual amount of cyclosporine. For adults, cyclosporine is usually given in cc per 100mg. This patient however was only supposed to receive 30mg which would have translated into 0.3ml. Because I was so used to giving whole cc/ml, instead of drawing up 0.3cc/ml I got 3ml/cc (10 times the dose). We did not discover this error until the next day when the patient's cyclosporine level had skyrocketed. My nurse manager questioned me about the dose of cyclosporine I had given the previous day, it was only then and there that I realized what mistake I had made (no patient harm was found). Some of the changes that I have seen come into practice have some from near misses! For instance, our Heparin flushes and potassium chloride were in very similar bottles, and it was not uncommon at that time to have vials of both on the top of the mediation cart. My coworker was drawing up what she thought was heparin to flush an HL, when in fact what she was drawing up was potassium chloride. I happened to be standing there when she was drawing this up. She started to walk away from the med cart and I asked her where she was going, she told me to flush the HL. I said not with potassium you're not. She almost fainted (one cardiac arrest avoided!!)! I reported the similar vial problem to the pharmacy and we started using a different form of heparin. We also in the meantime stopped stocking heparin and potassium vials on top of the med carts throughout the medical center.

My second semester of nursing school was horrible. I made an error and I still have not let it go and question where I stand and if I am meant for this field. Your words really resonated with me.. Unfortunately, yes we are human and truly we makes mistakes. It 's so painful and devastating.Don t want to justify myself but the system overload us with a ton of tasks to do.Should not be like that if employers can have a better organization on the tasks with more employment and I consequences, be a better career to love for more many. Support is the key too, Sean...you are right!

I made a vaccine error as a new nurse. It was harmless (gave Tdap instead of Dtap to a 6 yr old) but I will never forget how scary it was. I am so glad it wasn’t a different medication that could have hurt the child. I learned to slow down and try not to let stressful distractions lead to cutting corners. I still feel guilty when I think of it. It is a terrifying feeling wondering if you wil lose your job if you confess. That’s why just culture is so crucial. Hopefully I will continue to prevent future errors.

The font/print size on IV medication labels that nurses give is extremely small. This has huge potential to cause medication errors. Why is nobody fixing this problem?? It's unbelievable and continues year after year to be a MAJOR problem.

If a person follows protocol and scans the med and the patient's armband, the chances of med errors are GREATLY reduced--unless a person willingly choses to bypass protocol. By doing that the nurse now takes it on themselves if they make a medication error. THere was one case I scanned the med and the patient and it was correct, but the Zosyn container did not look right. So I called pharmacy. It happens pharmacy made a huge mistake and mislabeled the drug. But that is rare but I"m thankful I caught it since the patient had some drug allergies to antibiotics. But I did get scared afterward--errors can happen and it was not your fault.

I accidently forgot to give medications at a certain time, somehow I missed those meds.. idk how!! I'm still under preceptorship but omg my stomach hurt. We find out the next day that I had missed those meds. The supervisor asked me about it, I told her that it was an honest mistake. Me and my preceptor missed it. I wonder if they can laid you of for this?

Anyone can correct me... but you don't verify dose changes/boluses at the bedside for that medication (when used per provider order). I wasn't doing anything that veered from the protocol or order. I made an error with the buttons I pushed on the IV machine.

I was supposed to give a direct iv injection..I inserted the needle and withdraw blood and slowly started pushing the medication but after some seconds it started to bulge and the patient was in pain..so I stopped the injection..The patient was in severe pain..is this an error..this terrified me..what am I supposed to do???😪

No, don’t worry.. it happens possibly because of patient being sensitive to that med if you got a good blood on aspirating after IV placement or may be vein is too small like a capillary

They don’t see it that way. I’ve been a nurse for a while. I realized this morning that I missed an IV Antibiotic dose yesterday morning. I’m the only RN on nights. It was on another station. I reported the error immediately! The doctor went off on me via text message. He called my boss. No harm came to the patient. Now I have to meet with her this coming Thursday for a write up🤦🏾♀️. I was told by the assistant D.O.N that “you should never miss anything!” I’m torn. I came clean and told the truth and I’m in trouble. If I would have tried to cover it up and it was discovered later, I would be in trouble. I can’t wait to get out of nursing.

Thank god it was an opioid that wouldn't cause any harm. Especially on intubated patients. Prefer fentanyl over ketamine for this reason.

I recently complained to this LVN supervisor because he was wasn’t giving me my meds every 6 hours . I’m having kidney surgery Tuesday and I’m not allowed to take blood thinners . Friday he put a blood thinner in with many other pills. So complaining to his supervisor he’s now taking revenge. I did not take the blood thinner or he other pills. A nurse told me I could bleed out while having surgery and die.. this recovery hospital is horrible. I would leave now but I had my left foot and partial leg amputated. I considering calling the police and at least file a complaint. Nothing happened to this person when I complained. He apparently feels comfortable trying to kill me.
