Here's one that tops all of them all. Well, this isn't a Mario game, but it has a character from the series in it and was made by Nintendo, so I'm counting it. In the Wii game, Captain Rainbow, you play as a superhero going on quests for different Nintendo characters, mainly obscure ones. One of them is Birdo who is in jail for using the women's restroom, and her mission for you is to go into her room and get her viberator to prove she's female. Unsurprisingly, this game never made it outside Japan.

Also in Captain Rainbow Birdo gets caught walking out of the womens restroom and gets thrown in a cage because the guard was swearing that Birdo’s a male. So Birdo asks you to go to their house to find something to prove that they’re a female. So you see something vibrating under a pillow and you collect something that the game never tells you what it is. So Birdo is not only a female but uses a vibrator. Got it Also Birdo is female.
