Besides Clomipramine, venlafaxine is what helped well against anxiety and panic attacks. Unfortunately, I had to stop taking these as I developed extremely high blood pressure. Have also tried duloxetine, also with good results. The problem is that I sweat a lot basically, and that these drugs increase the ability to sweat. But I would do anything to try clomipramine or duloxetine (now that the blood pressure is under control with bisoprolol and amlodipine). Unfortunately, my new psychiatrist is obsessed with vortioxetine and thinks it helps with everything. The positive is improved cognitive ability, but the anxiety and depression don't help much

I would encourage any patient to be assertive with their prescriber and really insist on a collaboration when it comes to settling on the medication regimen. No one prescriber’s zeal for a particular medication or combination of medications should overrule what the patient himself or herself experiences and prefers. The prescriber is there to guide and provide an expert opinion and to prevent toxicity, not to dictate treatment. There are many right ways to do things in psych; rarely is there only ONE option. You are the one taking the medicine, after all.

Dr. Silva; in the context of fibromyalgia, is tramadol equally effective for psychogenic pain as it is for somatogenic pain?

I think it would be impossible to make a blanket statement about the efficacy of that medication in any circumstance. Pain and pain thresholds are inherently subjective. There would certainly be individual variation but also variation according to dose and myriad other factors. I doubt that there’s an established answer to your question.

Dr. I have been taking duloxetine 60mg for over 6 months now but I haven’t seen my psychiatrist for 3 months now. Can I just stop taking it? I feel weird after a couple of days of not taking it.. but can I just stop? I started taking it because of chronic pain and anxiety after my grandmother passed away. How can I get off of this medication?

Definitely make an appointment to see your prescriber, as you are likely to get a physical withdrawal if you suddenly stop taking 60 mg (and of course, possible rebound pain and anxiety). You need to taper the dosage down to 20 or 30mg before trying to stop altogether.
