Beautiful piece and deep thinking, Ewan. I believe that the cause of our physical suffering originates in the suffering of our spirit trying to communicate to us through daily world and bring our awareness to its suffering by material manifestations because we have stayed separated from our spirit and its more subtle messages for too long. If not addressed, the messages tend to get stronger and more violent over time. The way to resolve the issue is to fully acknowledge the suffering we cause to our soul and spirit when ignoring them and living separate lives in our thoughts and imagination. The next step is to bring the awareness of the spirit into our daily life, no matter where we are and what we do, paying attention to our body, emotions, subtle feelings, our immediate environment and the consequences of all our thoughts and actions, paying attention to feelings of others etc. Whenever we notice that we have drifted into our thoughts or imagination and left the awareness of our present and our body behind, we bring it back and root the spirit inside, always being aware how our body and mind feel, no matter how painful the present situation might be, we want to make peace with it and keep on living because its taking us through the life like a river, and not every turn is soft and mild. Eventually, when the spirit is always fully present, life becomes joyful, inspiring and rewarding and we realize that all the turbulence we caused ourselves. Sorry for the long post.
