Dr. Corey Glenn discusses several factors that can affect the accuracy of guided surgery and provides tips on how to manage them. He explains that drilling depth, the amount of space between the guided drill and guide tube, and the height of the guide tube all play a role in accuracy. The speaker also highlights the importance of having an accurate CT scan and STL model and addressing factors such as incorrect syncing, inaccurate printing of the guide, and improper guide seating to ensure accuracy during guided surgery. 00:00:00 In this section, Dr. Corey Glenn discusses the factors that can affect the accuracy of guided surgery and how to manage them. He explains that while guided surgery is highly accurate, there is some play in the system that can affect accuracy. The three factors that affect accuracy are drilling depth, the amount of space between the guided drill and the guide tube, and the height of the guide tube. By measuring the dimensions of the drill and the guide tube, he is able to calculate the potential deviation and how it is altered by drilling depth or other factors. Knowing the maximum angle of deviation allows for extrapolation down to the length of the drill, and thus better accuracy. 00:05:00 In this section, the speaker explains the factors that affect the accuracy of guided surgery, specifically the drilling depth and the space between the guide tube and drill. As the drilling depth increases, the potential deviation at the apex of the drill also increases, which can lead to inaccurate implant placement. Additionally, a tighter sleeve around the drill results in less deviation, while more play between the tube and drill increases the potential deviation angle. The speaker also advises against drilling a big deep hole and placing the implant to that depth, as it can increase the risk of deviation. In cases where there is a significant amount of bone reduction to do, it is better to lower the bone first and position the guide tube closer to the apex to minimize deviation. 00:10:00 In this section, the video discusses the final factor affecting the accuracy of guided surgery: the height of the guide tube. Increasing the height of the guide tube results in a longer guiding surface between the guide tube and the drill, leading to more accuracy. However, excessive height, especially towards the occlusal aspect, can result in difficulties placing the drill into the tube in the mouth. On the other hand, the limited amount of guide tube height apically can also create impingement. Other factors affecting accuracy include having an accurate CT scan and STL model. 00:15:00 In this section, the speaker discusses various factors that can affect the accuracy of guided surgery. These factors include incorrect syncing between the STL and CT scans, inaccurate printing of the guide, distortions in the scan due to movement or scatter, improper seating of the guide, deviation from fully guided implant placement, inaccurate bone segmentation, and excessive guide spacer. To ensure accuracy during guided surgery, it is important to address these factors and to visually verify that all cusp edges are completely seating within the guide.

Great information Dr Glenn. We’ve been struggling with drill depth accuracy on the z3d drill kit from zimve. Would it be possible to have a call with you or send you some information to see what the issue is.

@cory, also a factor is the bone density variation across the diameter of the intended path of the drill.

Hey @cory! I just had an idea pop into my head. It might be useful to add holes on the sides of the surgical guide at undercut areas and then like you would take a temporary/provisional; crown matrix impression, add clear bite registration material or light body to the sx guide fully seat it, and it could keep your sx guide more stable throughout the drilling sequence.
