Dr. Gillard. Thank you for this video. I know this is an old video. Also after analyzing this whole thing I'd say one thing. Annular tears can be quite problematic and cause neurogenic symptoms without any nerve impingement. As you already mentioned the stuff that comes out of these tears is highly inflammatory and that irritate the sinuvertebral nerve and sympathetic trunk? My understanding the solution for annular tears is either Artificial disc replacement (which i don't advocate) or Fusion. Do you think it's worth Bill getting a discogram? Also, can you please advise if you have had any contact with Bill and how he's doing now?

Hi Doctor, when you have a bilateral pars fracture at L5, it becomes detached at the Pars but is the spinous process still connected at S1?

Yes, S1 will be fine. It's the L5 posterior arch (specifically, the pars interarticularis) that is the potential problem.

@@douglasgillarddcprofessoro5285 Yes, I was just wondering if the bilateral fracture meant that it was completely detached from your spine and only held together with the muscles and ligaments or if the bottom part of the bone is still connected to the S1.

Hope have a cage to enhance fusion

well that's what i was thinking. Potentially ALIF cage at L4-L5 to avoid any further posterior scar tissue adhesion. But again I could wrong suggesting an anterior approach in this case

good luck to you. I'm no doctor, just another bad back sufferer. but after hearing Dr. Gillard discuss your case in details, I strongly suggest you get to a good spine surgeon and get those screws taken out. Dr. Gillard says they could be life-threatning. But he says he told you all this.
