Okay hi fan of the show who watches it constantly here- Everyone can understand stewie. Said by Seth MacFarlene, creator himself, and there are many many instances of the characters responding to him yes even Lois. So they all UNDERSTAND him, they just ignore him and don’t take him seriously because he’s just a kid. So they all understand him but whether or not they choose to respond to him is just simply whatever is better for the episode.
