Actually we do know. Seth MacFarlane and Butch Hartman worked together on an episode of Johnny Bravo where Adam West played an eccentric version of himself who tries to help Johnny solve the mystery of his missing Momma. Butch went on to make Fairly Oddparents, Seth went on to make Family Guy, and both shows had Adam West playing crazy versions of himself (Fairly Oddparents had a team of orderlies chasing Adam West with a big sedative needle). So there you go, it started with "Johnny Bravo Meets Adam West!" in 1997.
