Also maybe more leagues from countries which have a league in the game, it would be really nice if want to be in your country, manager or player, to start from the inferior leagues and promote into the superior ones.. luckily there are some mods of inferior leagues from different countries.

Their should technically be two versions of San Siro, San Siro when I think Milan host, and Estadio Giuseppe Mezza when Inter/Italy hosts.

Yeah but still the same stadion. Outside off Italy everybody calls is San Siro because it's the official name now. Only Inter and yes I love both Milan teams. But still its not that big off a deal. I wish we got more serie A stadiums. But if I remember right in previous fifa if Inter was at home it was Giuseppe Meaza. But now not anymore. But at least both teams are partner clubs with EA.

I’m not too sure when licensed stadiums were introduced but it has been a very long time. As for your other question , the Championship and League 1 teams have licensed stadiums because they are either very historic teams or teams that have relatively recently been in the Premier League, whereas, no current league 2 teams have.

The reason Portsmouth and Sunderland have their stadiums in the game is because they were in the Premier League. All teams that played in the Premier League will have their stadiums in the game no matter how far down they drop.
