Secondly if we involve double tendon TP+ PERONEUS LONGUS for foot drop then after cutting PERONEUS LONGUS and attaching with anterior tibialis with TP rest of PERONEUS LONGUS do you attach with PERONEUS braves or just leave it ?

Is it compulsory to transfer PERONEUS LONGUS with TP for foot drop or can you also operate only TP transfer for foot drop because if PERONEUS LONGUS muscle working fine then why should need to reconstruct with TP tendon for foot drop, I know that provide more strength for dorsiflextion if we use two tendon but same time I am thinking for losing evertion and plantar flexion due to involve PERONEUS LONGUS transfer

Tibialis posterior is the strongest tendon available and the main tendon which can provide dorsiflexion after transfer. I am not using peroneus longus now days as I feel tibialis posterior is strong enough to give adequate movements.peroneus longus I use in selected patients who have weak planterflexion.

@@dradnanfirozkhanorthopedic2534 is there any problem in future after TP tendon transfer such as PTTD (FLAT FOOT) ? because TP is reconstructed so how to deal with FLAT FOOT and inward ankle rolling

@@hanman3335 yes you are right. But flat foot doesn’t cause much of a problem . Pts are rather happy with a mild flat foot deformity rather then a drooping foot.
