It was never said. But I imagine he was bitten by a werewolf...and may indeed not have known it. He likely thought it was a bite from a coyote or even a wild dog....sought treatment & never gave it another thought.....until!

Actually in the Stephen King novella, The Cycle Of The Werewolf, on which Silver Bullet is based, Lowe becomes cursed when walking through a cemetery and the spirit of a Werewolf enters him.
