I'm not a doctor but it sounds a bit like he had Red-S osteoporosis. Hope he will be okay in the race.

It does sound like osteoporosis. It's quite possible to continue to run ultras with it (I do, although as a very, very average runner) but you do need to take care to eat enough and get enough vit D.
