Actually no. In the Lore they are not related. Not even "adopted" like Ainomoto claims. She basically only sees him as a big brother FIGURE. That's why she calls him big brother. Anyway. If you don't know. This is "Hetalia" every character is the personification of a country (so yes it's a show making a lot of jokes about stereotypes). Which makes it kind of hard to tell if they are related or not. The "East Slavic" countries of Russia, Ukraine and Belarus consider each other siblings. Russia calling Belarus and Ukraine sisters and they call him brother. But by this logic. Germany. Austria and Switzerland would be siblings. After all. They are all german speaking and culturally closely related to each other like Russia, Belarus and Ukraine. But they never called each other "brother" or are refered to as "siblings". Switzerland considering Austria even his rival.

Well... Hetalia's old media was very controversial. Like this video is so vanilla compared to them. But Himaruya decided to change the personalities of them. So, fandom doesn't count those problematic things from old media anymore. Also! You may have noticed that Switzerland is so calm even while his private life is violated by Hungary. In canon, his personal borders are very important for him. He even carries gun to protect it. So I guess it may not be canon.
