If you think about it, except for a few occasions, back in the day, when Mike wanted to have a serious father-son talk with Sonny, the mob boss only gets referred to as Michael when he's getting arrested, then he's Michael Corinthos, Jr. Calling Carly, Caroline, was one of the ways that Nurse Mary Pat taunted and tormented her. Over the years, it seemed to be Luke's exclusive preserve to think of and refer to her as Caroline, initially not meant kindly.
