The people in Inazuma call Ei "Baal" since they never knew that Makoto had a twin and that she died. In the story Ei states that she was originally Baal's "shadow" or body double. Since the original Raiden Shogun was killed, she took over that title as well as the name "Baal" So calling Ei the "Raiden Shogun" is the same as calling Ei "Baal". She took both titles/names.
