Qantas Flight 473, flown by Pilot Max Smith and Copilot James Robinson, after ATC allows them to take off from Japan to Singapore, ground crew forgets to fuel up the engines and pump up the wheels, leading the plane to start slipping and overrunning the runway..it started stalling as soon as it took off, making it crash into the water, however, only 30 people survived. (it was April 12, 1994 3:30PM.)
