Goku first transforms when Emperor Pilaf seals him and his friends inside a chamber designed to amplify the sun's rays, effectively cooking them alive when the sun rose, but a full moon rose first so Goku proceeded to go on a rampage destroying Pilaf's Castle.

It is for the second time he turns into a great ape. The first time was when he killed his grandpa by accident.

Grandpa Gohan must have seen Goku turn into an Oozaru before in order to tell him about the Oozaru, so this is at least the third time. Goku's early life with Grandpa Gohan and turning into the Oozaru would be a good prequel movie. It is odd that Goku never put it together that he was the Oozaru until his battle with Vegeta.

This is not his first or second time. Also, he is not turning into an ape, just a bigger monkey, like how Master Roshi did not turn into a monkey the first time that he was shown using the Kamehameha, just a bigger ape.
