I see that you start your downswing with your arms. Should be your hips. What works for me really well is stop at the top of your backswing. Turn your hips toward the target without moving the club and then swing. It doesn't have to be dramatic just what is comfortable. It might feel like your coming over the top but this will show you the feel of separation and allow you to trap the ball. You will know when you get it right because the impact will feel very solid with what seems like very little effort.

Honestly i think your problems start at address with your posture. You look off balance which believe it or not will create loads of other problems. But my advice would be to go get a lesson from a pro and tell him you want to first off make sure you aren't making any errors on the setup, address and posture. Might surprise you
