there is alot mobile apps and low quality sites under the where your ads shown section and if we exclude them or only add high quality sites in placement section does it result in higher cpm

if you create a campaign with goals then google strict some targeting options in the campaign That is why creating a without a goal, campaign with goals this options are given for beginners to create a campaign easily and don't get confused

Yeah it doesn't matter, but if you want to have the data outside of the G-Ads you should use parameters.

With the current algorithm you should either know what your exact goal is as the system will give you specific bidding strategies, if you are not entirely sure of the exact goal create a campaign without a goals guidance is the best way to move forward as it gives you all the available options.
