You should have few detailed blog posts/pages with quality content and other important pages along with a nice and working menu to get approved. if you think you have done it then apply for adsense, in worst case you will know what is missing and you can apply again. There is no limit to apply!

It is clearly written above $4272 "Your Potential Annual revenue" on the page you are showing. That is $356 per month on 50k views per month. hey , in starting it is recommended to disable it. Because it is possible in some cases that it is breaking the page to the robots. Rest you can check your pages preview if they are loading fine then you do not need to worry about disabling the plugins.
