Do you have a tutorial for this that's a bit slower and more basic? I'm having a hard time following because it seems this tutorial assumes the form builder and all the layouts are already familiar to the user.

So it seems like this process is only for forms that clients fill out, either because they've gotten the form from your website or you've emailed the form to them. Is that correct? I will have clients fill out a brief screening form before scheduling an intake. However, I don't have clients fill out the intake - I do that during the first session. How do we create a form for ourselves that we fill out as we are talking to clients?

You can use forms for you to fill out as well. I will say this. If you walk away from from computer and when you come back your browser refreshes, you may lose some data in a half-filled-out form. You just want to make sure that you fill out your forms fully, in the moment, if you're using them for documentation.

Thanks for this. Is the form developer option also hipaa compliant? Is having people type their name qualify as a legally binding signature? Is it allowed by insurances (specifically Medicaid)

I'm not sure about form developer. I've tried reaching out to them in the past to no avail. And you'll have to do your own investigation on the esignature question. I recommend either reaching out to a lawyer, or looking up this information on-line. Good luck

When I selected the add-on from Google Form, a pop-up comes up that says: "Form Publisher -Form to PDF-" Help" According to comments on KZbin, this has been an ongoing issue for the past month. I have submitted a "support ticket" a month ago, and there has been no response. It's no surprising seeing all the negative reviews. I'm not going to trust my business to something this unreliable (especially when we have to be HIPAA-compliant), and I recommend any business owner to stay far a way from this unreliable and potentially UNSAFE add-on.

Yes. The $6 plan. You must sign a Business Agreement, or BAA, to be in compliance. Google how to do this, and it will walk you through the steps. Have you already purchased the basic plan?

Yes, this works with the basic plan, also. Form publisher is HIPAA complaint if you sign a BAA with them. This was a little bit of a process. In truth, many of these discussions take place on our Facebook group. It will be easier for you to get more regular tips/tricks by joining that group: Gsuite for Healthcare Providers.

what about for a regular patient revisiting? because then they will have to re-enter their name, date of birth etc.

That's a good point. In this case, you might consider a separate form because you've already got all their demographic data, but you will be curious about any new emerging information. So yeah, I would make a second, abbreviated intake for that purpose.

Is there a reason you can't just "print" the google intake form and then have that for your medical record?

You can absolutely do that if you’re keeping hardcopies of everything. You can have it set up so that you can print directly from the spreadsheet that the form populates, or you can use form publisher to have it populate a Google doc and then print it from there. That’s what I would recommend.

Tracy Wright Oh, I thought you were keeping paper charts. Totally, you can do it that way. Very cool. There are other platforms out there that will do this, but I feel that Form Publisher + Gsuite is the cheapest and quite efficient. Yeah. I tried it. The only issue is that I often don’t finish notes on the spot, so when I get back to my computer the page refreshes and I lose all my data. I like to use Text Blaze to fill doc templates as I go. (Got another video on that). I primarily like Form Publisher for populating parts of Eval within a Google doc. After that, I just duplicate notes for future visits.
