It's not the "gear", it's continuous cycles of antibiotics that his idiotic and uncreative doctors keep overprescribing because they are ignorant of the cutting edge therapies available for people with autoimmune conditions and horrible GI issues. Androgens do not cause gut issues. Oil based drugs don't even go through the gut and testosterone increases the immune system and your ability to handle stress. It's the long term effects on the cardiovascular system that is the issue.

Orals can be harsh but they don't destroy gut bacteria either. He probably came off for long periods as well and saw no improvement in his stomach issues.
