Yes, there was hardly any build up, so it coming out of nowhere was surprising/nightmarish (and the music further adds the creepiness). The sight of the Biolizard is a tragic, yet deformed monstrosity. Sonic and Shadow did it a favor by giving it a mercy kill. There was a scene where Shadow confronts Rouge and she’s holding a newspaper that shows the Biolizard (but most players probably didn’t notice).

To be honest I think the biolizard is a pretty good villain and pretty underrated in my opinion! Of course it's no one of my favourites my favourites are Mephiles and Solaris but I still think the biolizard fits perfectly as a final villain and had alot of potential of being the ultimate life form for the villains

TL;DR, yes, they're boils. According to the Sonic wiki, the boils are a result of the combination of the destruction of its life support system (the device on its back), the harsh conditions of outer space and atmospheric entry tearing away at it, and its need to draw power directly from the ARK. For some reason Eggman refers to the boils as "chloroplasts", though that's likely incorrect as I highly doubt this thing is capable of photosynthesis (plus they're usually green, not pink).

I mean say what you will but biolizard really was closer to being a ultimate life form than shadow. I mean it at least sports invincibility and can drag the entire colony ARC on a collision course to earth while lasting multiple minutes in space without it's Life support system and still tanking a few hits from Super Sonic and Shadow. Sure it needs a life support system but like... what does Shadow have to offer? The only thing he has the Biolizard doesn't is super speed. Even when it comes to energy attacks the Biolizard can clearly do that too as well as altering gravity. I mean which other Boss in the entire Series can truly claim that they have invincibility on paar with Biolizard? The only reason Super Sonic and Shadow could even damage the Biolizard in space is because of the boils that formed on it's body as a result of missing it's Life Support. Attack anything else than the Boils and it does nothing. Literally more tanky than any of the ancient gods that were summoned in the entire Sonic franchise.
