It's always safer to go less cos of the risk of necrosis due to vascular compromise. I think this doctors approach is perfect. I would rather go less than sorry.

Dr. Kotler here. Anjali, you said it right. Step-by-step is wise and has worked well for our patients for over 40 years. If there were a somehow " better way", I would use it. There is no safer way.
