The big hand could be a vision of the True/Fallen Creator...but honestly I'm not sure, could be a lot of things. The last scene of the fog world on fire might be the part where Klein Divined the Sun and was protected by the fog world cause it took the brunt of the attack. The scene with Lennard is most likely the entrance to Trissy's dwelling. The first time they went to fight her along with Dunn and Klein. It was a garden after all and the massive hand might be the scene from later on in the book where some demi gods were in battle but that's a reach.
