I have a question: on the Educational Background, I would like to provide a separate sheet for the schools I have attended in College. I attended 3, where some of the courses were credited as well. Sir, since I am adding a separate sheet, how should I print it? I am planning to have it notarized. Since it should be "back to back", should I print it Page 1(FRONT: complete, BACK: only the separate sheet for educational background), then Page 2(FRONT)-Page3(BACK), then Page 4? OR, Print it as Page1--2, then Page3-4 then print a separate Page 1(for educational background)? I hope you can answer this, thank you.

Simply print the standard PDS back to back. For the separate sheet, print only page 1 and then fill out just the additional info that didn’t fit on the main PDS.

Voluntary Work or Involvement in Civic/Non-Government/People/ Voluntary Organizations • Indicate the FULL name and address of the organization where involved as voluntary worker. • Inclusive dates, start (from) and end (to) should be in numeric format: mm/dd/yyyy. • Indicate the number of hours of voluntary work rendered. • Indicate the position/nature of voluntary work rendered." Do I need to have certificates to prove my volunteer involvement? What if I am currently still involve in the volunteer should I put my hours? What if I don't have certificates to prove that?

Do I need to have certificates to prove my volunteer involvement? *Most often, the certs are not needed since VI do not count in the points, etc., but it is better to be always prepared with the certs just in case the prospective employer will nitpick and ask you proof. What if I am currently still involve in the volunteer should I put my hours? What if I don't have certificates to prove that? *You can estimate the hours, or just put N/A, then don't force yourself if you don't have certs.

OJT is not considered work experience since it's part of your academic training, but if you don't have experience yet, you can include that in your PDS for now, no assurance it will be credited as "work experience."

All trainings should be placed under VII.30. If kulang ang space, use a separate sheet. Get another Page 3 of the PDS and fill out only VII.30. Make sure that you have certificates to back up the trainings you mentioned, but most importantly, include the most relevant only. MANY TRAININGS WOULD NOT MATTER IF THEY'RE NOT RELEVANT TO THE POSITION.

Under K12, you graduate from high school when you finish Senior High. Finishing Junior High is just "completion," not graduation.

If it's an "additional sheet" for Educational Background, then open/print another sheet of Page 1, then just fill in your additional educational info under III. No need to fill out the other info since you already should have done that in the "first" sheet of Page 1.

Usually, your signature has to be affixed after filling out the entire PDS. As to who will sign below depends on the agency you're applying in. Most of the time, it needs to be notarized before submission. Best to ask the agency what their policy is.

It's focuses on Teaching methodology such as classroom management, learners engagement strategies so on. Di ko kc mailagay sa training kc matagal na course to 1 year. Pero natapos ko in 4 months kc self paced. Need to finish 8 courses plus 2 capstone project. At dapat pumasa yong projects mo sa Expert Review Bago ka bibigyan Ng Certificate from Arizona State University, USA with their office zeal.

If your relatives are not your future bosses in that agency you're applying in, then answer NO to #34.

@@Popustusness Supposedly, they don't count since they're part of your academic phase. If you don't have any outside seminars yet, then you can put the college seminars in there for now. As you gain experience, slowly replace them. Your prospective employer will not count the college seminars though.

It really depends on the agency you're submitting the PDS to. Make sure to ask if they'll accept a PDS with E-signature. Otherwise you have to print, sign, scan and send.

I'm really having a hard time po kasi hindi po siya magkasya dun sa alloted space at sabi nila bawal daw po lahikan ung space if computerized po ung pagfifill-out. Pero nakaindicate na man po dun sa mismong PDS na maaaring gumamit ng separate sheet kung kinakailangan, paano po un gagawin, sir, 'ung paggamit ng separate sheet? Sana po, sir, masagot po ninyo. Maraming-maraming salamat po.
