Arcana Otyugh: Can "belch" once per turn, immediately instituting an arcane spell mishap on anyone within an area around it! Every time you cast a spell at it, the belch steps up a Tier on the mishap charts! (That'll teach them to wall of force things indiscriminately)

Honestly, I'm fine with players that hit higher levels not having the world constantly scale with them. The player earned that strength by keeping that character alive, If a player gets bored with how strong their current character is, have that character reflect that and retire from adventuring. That being said, if characters die, then players just make a new characters at the same level, I guess that presents a problem lol. I almost think that drastically slowing leveling progression, or forcing players to lose a level upon death might slow the outcome you describe.
