In today’s moments before disasters: How the faq did Miko even end up pouring lava while she was on a minecart? 11:02 shows this. Its pretty quick, so you might wanna watch this moment at 0.25x speed. Miko was somehow holding to a bucket of lava on her left hand all the way. She went on the platform, and selected the minecart with her right hand to place it on the rails. Hence her right hand was empty and there was no way lava was gonna come from there.. But, as she was riding downslope from the platform, my guess is that she pressed “shift”. Which activated the item in the left hand, according and you could see the lava was deposited in slow motion at around 11:02, on the platform..... *edit: A viewer below corrected me, as I'm a bedrock player and I can't put stuff in my left hand other than shields.....

[shift] only makes you crouch or leave any transport you are in (the minecart in this case), to place the lava from the left hand, you only need to [right clic] on any surface (the same as with any action with the right hand) My guess is that she wanted to go the other way, so when she noticed, she tried to leave the minecart, first by right clicking (very likely by mistake) then she shifted. Since she was carrying lava on her left hand, when she right clicked, the game placed the lava, since her right hand was empty and she wasn't facing to any interactive block/entity.
