On the one hand, looking at this from the game perspective and not fandom, it's a total Darth Vader moment, scoring the previous villain for not being able to do their job while he strikes fear into all. On the other hand, Mono would've held out hope for six. Until the tower caused him to snap. It's honestly heartbreaking how the tables turn from her uncaring attitude and his love, to her obsessing over what she didn't realize she had before, and him accepting that it can't be the way he hoped.

Mono (as Thin-Man) was right from his point of view towards Six (the Lady), how she betrayed him after all the things they'd been through together as children trying to escape Pale City, and it injuried Mono's heart emotionally... if she didn't let go things would've been a better outcome but instead she unknownly repeat the time loop. Mono was deedly afraid of being alone and how the good deeds he done for Six ended with nothing, his loneliness turned into unbridle anger of Six's selfish betrayal as he spent so many years completely alone inside the Signal Tower as every ounge of his humanity slowly dies until he becomes a Shadow of his former-self as the Thin-Man.

However, there is still good left Mono's tormented shell... that he couldn't bring himself to kill her out of anger, it would make him feel more empty and lonely as he was as a child, even if her betrayal still hurt him after so many years of eternal solitude, deep down Mono is still the kind boy he used to be. Killing Six would never bring him peace despite the betrayal he suffered, everything that happened between them, Mono will never bring himself to kill her for revenge, instead taking misery by letting go of her neck proves that there is still humanity left in his heart outside of his corrupted form as Thin-Man, after all taking her life away would never bring back what he'd loss.
