For the hemodialysis one, why wouldn’t you slow down the dialysis? I know you’re supposed to assess first, but if the questions gives you enough assessment data, I was told that you should perform an intervention instead of assess. By the patient saying they were dizzy and nauseous and we know that disequilibrium syndrome is a complication, we can deduce that the pt is most likely experiencing that. I always get confused when to assess vs intervene because so many times, I have chose an assessment answer and it was wrong 😢

Therefore, checking the blood pressure first allows the nurse to make an informed decision regarding whether to adjust the dialysis rate or pursue other interventions based on the patient's condition. :)

When you say “do at least 50 questions a day” what kind of questions would you recommend. I am currently in Med Surg 1 for LPN and our mid circular is somewhere in May. Our class uses the Nclex Book, FA Davis books, and evolve elsevier. What kind of questions would you recommend and from what website or book?

Hey Leo! Know your basic normal lab values for hematocrit, wbc, hemoglobin, potassium etc. Do at least 50 practice questions a day the week of the exam.

A helpful tip to prepare: Do at least 25 to 50 questions per day to prepare. You can find med surg practice questions on the quizlet app, or there are a couple of youtube channels that are also great and have practice questions. The more questions you do, the more content you will expose yourself to. It is important to go through the rationales and find out why you got the answer correct or wrong. So when you do your practice questions, do it yourself closed book and review each rationale after trying each question on your own first. You will do great! If you have any other questions. Just drop it here.

hi love I just found your account, I took the exit hesi and failed terrible with a 588. I need a 850 to pass and graduate. luckily I have until the end of April to prepare for my next exam. any tips on how to improve my score by 300 points :(

Hi Ayni! Congratulations on working hard to get to your last semester 👏 You have been prepared, and I know you have the knowledge to pass your exit hesi. I am sorry you didn't get the score to pass. Some tips I have are: 1. Thing about the last hesi and the rationales you saw. Write down topics or things you didn't know and get some more knowledge on it. 2. Do 20 to 25 Questions a day in test mode then review each rationale. 3. Focus on your nursing fundamentals and med surg. 4. Be positive. 5. On exam day before you go in get a piece of paper and make two columns. In the first column write down all your fears and in the opposite column write down the opposite of these fears for example: I will fail this hesi | I will pass this hesi. I will score 588| I will score over a 1000. Tear it up and throw it away and then walk in to take your exam. When you get into your exam if the have scrath paper write down all the positive things. After every 10 questions read the positive notes. Positive thinking helps us to relax in exams and keep our heart rate and breathing good or anxiety low. I believe in you and I know with some extra practice everyday you will pass this. I will also post a review for the hesi exit later this week.

You are almost there! Some other tips will be to do at least 25 to 50 practice questions each day. There are some great KZbin channels that have practice questions. One of my fav I like to suggest to students is Nexus Nursing. You can find lots of med Surg practice there. Also uworld is a great resource but does have a fee for access to the question banks. You will do great. Don't give up. Practice practice practice.
