Here I'd though I'd showcase this mysterious hidden face behind Anakin during his Mustafar duel in Episode III Revenge Of The Sith. In-Universe, perhaps it's Ezra as he walked past the halls of time in The World Between Worlds.
Out of universe, it's obviously a behind-the-scene blunder of either the stage crew or stunt guy.

Probably a masking issue if I had to guess. Someone must have been standing behind Hayden as he was jumping and in post production they forgot to mask him out.
