Not sure if this has been said already but here's an easy mnemonic for laterality of lesion involving uvula vs. tongue deviation: uvulA -> A = away / Tongue -> T = towards.

My dad had a hemorrhagic stroke on his right side which was due to chronic hypertension a berry aneurysm rupture in the midbrain. He did have a craniotomy to release pressure but he did have a midline shift and hematoma. The doctors and the neurologist really can't State what kind of syndrome he has or exactly what all has been damaged it's been a very frustrating for us family. He's is now quadriplegic. After surgery he was in a coma for about 3 weeks and then he started making more progress as far as conscious goes vegetative in between minimal conscious. Now he's in and out of it he's not really fully conscious ever but he can open his eyes sometimes he's got a tracheotomy but he can't move his arms and his legs. I don't know if he's locked in or if he just has severe significant brain damage and there really isn't an exact disorder to his condition. It doesn't seem to fall within any syndrome or disorder that I've found so far it's kind of a mix of a little of this and a little of that but nothing concrete exact maybe you can help me a little on what you think it could be. I would be so thankful for your input.

Rosalina, I am so sorry to hear about your father's condition. As far as your question goes unfortunately there is a divergence between what you may learn as a medical student, and what tends to happen in real life. In textbooks, boards questions, and academia, everything is straightforward. The syndromes depicted are always classically represented with features and catch phrases. In the real world of clinical medicine, as you will see on rotations and in residency, there is a grey area to these same topics. Many neurologists will sometimes be able to postulate based on MRI and clinical findings, where a lesion may be located. An example of this in the literature, may be decreased consciousness in the setting of infarction surrounding the reticular formation. I am wishing you and your family the best, and if you need anything, please don't hesitate to reach out. Thank you so much, Dr. R! After watching this video, I was promoted to Jr. Butcher at the Costco I work at! Just kidding. I started watching videos on the Titan sub CEO, Stockton Rush, then began trying to understand his thinking.. which led to a few Neurological videos. I work with Databases and Web Development in the real world. After 30 minutes of your presentation, it saddens me to think about how much I didn’t know about my own Brain.
