It's actually quite easy, usually Kaoru is draw more short than Hikaru even through they're in the same high, maybe to emphasize the seme and uke thing. Kaoru is usually the left one and Hikaru is usually the right one. You can also hear the tone and voice type changes between the two if you listen closely (I'm talking about the Japanese dub, and yes, I do can tell them apart in the sub version). Also Kaoru is usually more quiet and mature than Hikaru (He's also my fav between them btw-)
