I don't know what's worse. Running headlong into another train or being on a plane about to crash into a mountain. Probably the plane. At least it's good to hear Johnathan Aris narrating. Love that guy.

It's a recurring factor in ever so many accidents -- when safety systems are made too inconvenient and/or too annoying the people who have to live with them daily WILL disable them. The designers of safety systems need to spend time living with them personally -- operating buttons or switches, listening to nuisance alarms, etc. That way they'll find out what human factors they need to take into account in their designs. I know personally that the sewing machines in a factory I worked in were not equipped with needle guards because they would have made it not just difficult but actually impossible to do the work we were required to do. The designers had, apparently, never actually attempted to sew complicated items with the guards in place.

Better training is needed, the why of the system needs to be taught.
@mbvoelker84486 ай бұрын
@@Thedrek You can say that, but if the safety system is too annoying or if it makes the task too difficult to accomplish then the people who have to live with the annoyance will find a way to disable it. The solution is for engineers to design safety systems that don't drive the operators nuts with false warning after false warning 40+ hours a week.

Yes. Reminds me of the reverse gear beeping on vehicles bigger than cars, the beeping goes all the time the reverse gear is enabled, so road works etc you might have four of them going off at one time. The noise is so annoying it’s ignored and you get to the stage where you dont even know where it’s coming from so you switch off.

If you've never fallen asleep on a train before, it is literally one of the easiest things to do. There's the predictable rhythm of the wheels on the rails and the cars sway ever so suddenly so you feel like you are being gently rocked. The fact that the engineers didn't have a full day or night's rest is the cause here.

Independently from the lack of sleep the trainmen got the technical safety precautions of the train were insufficient. In Europe the later improved Canadian dead men pedal which can't get bricked was standard long ago, and there was a system called Indusi which works with magnets at the rails and which activates a forced stop of the train if it overruns a red signal and the driver doesn't react on it. This system was introduced in the 1930s and was used for 50 years before this crash happened. Both systems would have prevented this disaster.

We all know that Wayne Smith was lying. He was sound asleep just like the other two. However, management bears a huge portion of the blame for scheduling people into shifts that don't allow them to get sleep, not to mention keeping obviously dangerous people on staff. The thought of that little boy trapped and dying on that train hurts my soul; rest in peace to all the victims.

By law any job requires you to have at least 8 hours sleep before you can start your next shift,no matter what industry you work in.
