BSOD has actually been around since Windows 1.0, mainly caused by an incompatible version of MS-DOS.

Fun fact: Some bsod version of windows like windows vista, xp, 7 are EXACTLY the same. Although, windows 1 and 2 needs a compatible ms dos to install. And if ms dos is incompatible for windows 1 and 2, you will instantly get a bsod :P

Fun fact: before the Windows 8 BSOD was only on English. But in Windows 8, 8.1 and later it's on the same language Windows interface have. But early startup BSOD is still only in English.

Not always. Sometimes it happens because something went wrong but it's not a permanent problem.

That screens happen when an Incorrect DOS version is instaled. It also have beeps (and i tested it on a virtual machine).
